Millstream Underwriting Group Travel Policy
As previously advised, our Group Scheme was renewed for the period 10th October 2002 to 9th October 2003.
You are reminded of the Health Warranty detailed in page 4 of the policy and the requirement that if any of the following apply to you, a travelling companion or an immediate relative, the medical assessors must be contacted: -
- You / they were aware of any reason why the trip could be cancelled or curtailed or of any medical condition, which could result in a claim.
- You / they have, during the 12 months before this insurance started, suffered from any chronic or recurring illness.
- You / they were travelling against the advice of a doctor or in order to get medical treatment abroad.
- You / they have been diagnosed as having a terminal illness.
- You / they were receiving, recovering from, or on a waiting list for in-patient treatment in a hospital or nursing home.
- You / they were waiting for the results of tests or investigations, or awaiting a referral for an existing medical condition.
If any of the reasons stated above occur between the date the certificate wording is issued and the first day of your trip, you must notify the medical assessors on 0870 741 5325. Failure to do this could leave you with no right to make a claim, and may mean that you travel with insufficient cover. The insurers reserve the right not to extend this insurance if deemed necessary by the insurers, where the booked trip could be detrimental to your well-being.
It is important to remember to contact the Medical Screening Service to renew any endorsement issued by them, if appropriate.
